Illinois health department reports severe nursing home violations

The Illinois Department of Public Health (IDPH) has released the 2024 Fourth Quarter Report on Nursing Home Violations. The report details 305 violation cases completed in the last quarter of 2024, available on the IDPH website.

Six facilities were cited with "AA" violations under the Nursing Home Care Act, indicating severe breaches that resulted in a resident's death. Each facility received a $50,000 fine for these violations.

Berkeley Nursing & Rehabilitation Center in Oak Park was among those fined. Elevate Care North Branch in Niles and Elevate Care Waukegan also faced similar penalties. Goldwater Care Gibson City, located in Gibson City, was another facility fined. The Grove at the Lake in Zion and Morgan Park Health Care in Chicago completed the list of fined institutions.

Members of the public can file complaints against healthcare facilities licensed by IDPH if they suspect violations. Information on filing a complaint is accessible through their website.
