Illinois Guardianship and Advocacy Commission Human Rights Authority Northwest Region met June 7.

The regularly scheduled meeting of the Human Rights Authority - Northwest Region was called to order at 4:01pm by the Chair - Kacie Terranova. Introductions and greetings were completed during the sign-in period of the meeting. A quorum was determined to be present after roll call of members via Hybrid (WebEx & In-Person) and appropriate business could be finalized. DRM read the confidentiality statement.
Minutes: Executive and regular meeting minutes from April 19, 2022, were reviewed and moved for approval with no corrections by Ruth R., seconded by Brett W., and motion carried unanimously by HRA members present.
NEW BUSINESS
Agenda Updates/Changes:
Addition/Changes to the agenda were the following items:
a) Vote previous executive session minutes remain confidential ( 01/22,03/22,04/22)
b) Report #21-080-9012 will not be reviewed ( additional information received before meeting, needs to be reviewed)
c) HRA Committee Elections Today ( Chair, Co-Chair & Secretary)
Colleen M., moved to add the changes and accept the agenda, seconded by Ruth R., and motion carried unanimously by HRA members present.
Ratifications for Case Referral and Acceptance:
22-080-9012 – Tabled from the last HRA meeting until receipt of a signed consent and at this time a signed consent has not been received. Dr. Wade moved to decline acceptance of complaint, due to lack of follow through and receipt of a signed consent. The motion was seconded by Ruth R., and motion carried unanimously by HRA members present.
New Complaint:
22-080-9013 – The complainant stated being hospitalized for mental health treatment and the facility failed to treat medical issues that arose. Once the medical symptoms materialized, the entire hospital staff disregarded the complainant/patient and did not treat but accused the complainant/patient of manifesting symptoms and faking illness. After being hospitalized for almost a month, the complainant/patient was transferred to another location within the same medial system. DRM has yet to receive a signed consent. Dr. Wade moved to table until next HRA meeting while awaiting a signed consent. The motion was seconded by Colleen M. and carried unanimously by HRA members present.
22-080-9014 – The complainant/patient stated being hospitalized for untreated medical issues and the facility failed to treat mental health issues that arose. During the hospitalization, per the complainant/patient the hospital refused to provide a copy of the medical records or the viewing of medical records. DRM has yet to receive a signed consent. Dr. Wade moved to table until next HRA meeting while awaiting a signed consent. The motion was seconded by Colleen M. and carried unanimously by HRA members present
22-080-9015 – The complainant was brought to the hospital on an involuntary admission application but was hospitalized under false pretense and inappropriate documentation. Complainant also spoke with LAS and at this time complainant stated not wanting to move forward with an HRA complaint. Sang moved that the complaint be closed, due to be addressed informally and the complainant not wanting to pursue a complaint. The motion was seconded by Dr. Wade and carried unanimously by HRA members present.

Closed Session
Pursuant to the Guardianship and Advocacy Act (20 ILCS 3955/21) and the Open Meetings Act (5 ILCS 120/2), Dave C., motioned to enter close session at 4:24 m and seconded by Brett W., and carried unanimously by HRA members present. The purpose of the closed session is to discuss any case assignments, case progress, report responses and case closures (19-080-9017, 21-080-9001, 21-080-9003, 21-080-9004, 21-080-9006, 21-080-9011, 21-080-9014, 21-080-9015). The HRA returned to open session at 4:58pm, via motion by David C., seconded by Colleen M., and carried unanimously by HRA members present.

21-080-9001(Florence Nursing Home and Rehab) - HRA Committee voted to accept draft report and send to facility for a response within 30 days, moved by Dr. Wade, seconded by Dave C, and motion carried unanimously by HRA members present.
21-080-9003(UWHealth/SwedishAmerican Hospital) – HRA Committee voted to accept draft report and send to facility for a response within 30 days, moved by Colleen M., seconded by Dr. Wade and motion carried unanimously by HRA members present.
21-080-9014(UWHealth/SwedishAmerican Hospital) – HRA Committee voted to table draft report, pending Director’s review and will revisit and review at the next scheduled HRA meeting, moved by Colleen M., seconded by Brett W., and motion carried unanimously by HRA members present.
Accept Responses, Close and Publish Report:
19-080-9017(UWHealth/SwedishAmerican Hospital) - HRA Committee reviewed and accepted the facility response. The facility requested not to have response published. Dr. Wade moved to close the case, publish the report to the agency, seconded by Sang C., and motion carried unanimously by HRA members present.
21-080-9004(UWHealth/SwedishAmerican Hospital) - HRA Committee reviewed and accepted the facility response . The facility requested not to have response published. Dr. Wade moved to close the case, publish the report to the agency, seconded by Sang C., and motion carried unanimously by HRA members present.
21-080-9006(Crusader) - HRA Committee voted to publish the non-substantiated report, response not needed, post on agency website and close the case, moved by Sang C., and seconded by Colleen M., and motion carried unanimously by HRA members present.
21-080-9011(UW Health/SwedishAmerican Hospital) - HRA Committee reviewed and accepted the facility response . The facility requested not to have response published. Dr. Wade moved to close the case, publish the report to the agency, seconded by Sang C., and motion carried unanimously by HRA members present.

Announced that HRA member, Brett W., started a new job, HRA members present offered congratulatory remarks.
The individual interested in joining the HRA provided background information on self and stated will let DRM know if want to move forward with member application.
DRM shared news of an award won by a service provider.
DRM announced working on getting all HRA Member’s photo id cards to be used for site visits in the near future.
DRM announced annual sexual harassment and ethics training certificates of all committee members are due July 1, 2022 ( DRM will send email reminder).
