Individuals who are unable to access their 1099-G tax form online or through Teleserve and have not received it by mail can schedule an in-person appointment for pickup at a local Illinois Department of Employment Security (IDES) office. Appointments are available Monday through Friday from 8:30 a.m. to 9:30 a.m. or 4 p.m. to 5 p.m.

These appointments are specifically for the pickup of the 1099-G tax form. To schedule, individuals should click on the link provided for the local office nearest them. If there is a need to cancel or reschedule, they should use the 'Reschedule' button in the Booking Details of their confirmation email and select 'Reschedule,' not 'New Bookings.'

It is important to bring two forms of identification for identity verification when attending an appointment. Acceptable forms include a Driver’s License or State ID, payroll stubs, W2, or a utility bill showing name and address.

The 1099-G form may only be picked up by the individual it belongs to; it cannot be collected by anyone else. This appointment is solely for picking up a copy of the 1099-G form. For other matters, such as unemployment insurance claims, separate appointments must be scheduled by calling IDES Claimant Services at (217) 558-0401.
