The Illinois Department of Public Health (IDPH) has released the 2024 Third Quarter Report on Nursing Home Violations. The report includes 312 violation cases completed during this period, which are accessible on the IDPH website.
Four facilities were cited with "AA" violations under the Nursing Home Care Act in the third quarter of 2024. This is considered the most severe type of violation, issued when a condition or occurrence at a facility directly causes a resident's death.
Alden Lakeland Rehabilitation & Health Care Center, located in Chicago, received a $50,000 fine. Another Chicago-based facility, ALIYA on 87th, was fined $75,000; this amount includes $50,000 for an "AA" violation and $25,000 for an "A" violation. Allure of Pinecrest in Mount Morris faced fines totaling $52,200 due to an "AA" violation and additional penalties for a "B" violation. St. Paul’s Senior Community in Belleville was also fined $50,000.
